To date, the endomicroscopic features of the distal respiratory tract have not been adequately studied in lung diseases, including chronic obstructive pulmonary disease and chronic bronchitis.</p> <p><bold>The</bold> <bold>goal</bold> of the present study is to describe the endomicroscopic signs of the distal parts of the respiratory system in chronic inflammatory lung diseases such as chronic obstructive pulmonary disease and chronic bronchitis.</p> <p><bold>Methods:</bold> A total of 21 patients with emphysematous and bronchitic phenotypes of chronic obstructive pulmonary disease and chronic bronchitis were examined. All the patients have undergone CLE of distal airways or alveoloscopy during bronchoscopy. The most pathognomonic changes were evaluated on the obtained endomicroscopic images.</p> <p><bold>Results:</bold> For each studied nosological form of chronic inflammatory lung diseases, by careful morphometric analysis of a significant number of informative images, the most specific endomicroscopic changes were identified with the release of CLE patterns.</p> <p><bold>Conclusions: </bold>CLE of distal airways in patients with chronic inflammatory lung diseases allows visualizing changes in the elastic framework of the acini, as well as identifying pathological intraluminal contents, which can be attributed to valuable additional tools in a row of diagnostic methods of respiratory medicine.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность.</bold> Конфокальная лазерная эндомикроскопия (КЛЭМ) дистальных отделов дыхательной системы — уникальная технология, позволяющая в режиме реального времени прижизненно визуализировать структуры, содержащие естественные флюорофоры, свечение которых вызывается воздействием лазерного излучения с длиной волны 488 нм. На сегодняшний день эндомикроскопические особенности дистальных отделов дыхательных путей недостаточно изучены при заболеваниях легких, в том числе при хронической обструктивной болезни легких и хроническом бронхите.</p> <p><bold>Цель</bold> — описать эндомикроскопическую картину дистальных отделов дыхательной системы при таких хронических воспалительных заболеваниях легких, как хроническая обструктивная болезнь легких и хронический бронхит.</p> <p><bold>Методы.</bold> Проведено обследование 21 пациента с эмфизематозным и бронхитическим фенотипами хронической обструктивной болезни легких и хроническим бронхитом. Всем больным в процессе эндоскопического исследования выполнялась КЛЭМ дистальных отделов дыхательных путей или альвеолоскопия. На полученных эндомикрофотоснимках оценивались наиболее патогномоничные изменения.</p> <p><bold>Результаты.</bold> При каждой изученной нозологической форме хронических воспалительных заболеваний легких путем тщательного морфометрического анализа значительного числа информативных изображений были определены наиболее специфические эндомикроскопические изменения с выделением КЛЭМ-паттернов.</p> <p><bold>Выводы.</bold> КЛЭМ дистальных отделов дыхательных путей у больных хроническими воспалительными заболеваниями легких позволяет визуализировать изменения эластического каркаса ацинуса, а также выявлять патологическое внутрипросветное содержимое, благодаря чему может быть отнесена к ценным дополнительным инструментам в ряду диагностических методов респираторной медицины.</p></trans-abstract><kwd-group xml:lang="en"><kwd>probe-based confocal laser endomicroscopy</kwd><kwd>alveoloscopy</kwd><kwd>chronic inflammatory lung diseases</kwd><kwd>distal airways</kwd><kwd>chronic obstructive pulmonary disease</kwd><kwd>chronic bronchitis</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>конфокальная лазерная эндомикроскопия</kwd><kwd>альвеолоскопия</kwd><kwd>хронические воспалительные заболевания легких</kwd><kwd>дистальные отделы дыхательных путей</kwd><kwd>хроническая обструктивная болезнь легких</kwd><kwd>хронический бронхит</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Thiberville L, Salaun M, Lachkar S, et al.
