The incidence of rectal cancer tends to increase, and surgical resection remains the main method of treatment. Anastomotic leakage significantly increases the postoperative mortality. The most effective way to prevent severe complications is the formation of a diverting stoma. However, the indications, the choice of a specific technique and the timing of the stoma elimination are not clearly defined. This review discusses the state of the problem, special attention is paid to the choice of the level of stoma formation (ileostomy or transversostomy).</p></abstract><trans-abstract xml:lang="ru"><p>В обзорной статье освещается проблема формирования превентивной кишечной стомы при резекциях прямой кишки с первичным анастомозом. Заболеваемость раком прямой кишки имеет тенденцию к росту, а основным методом лечения остается хирургическая резекция. Несостоятельность первичного колоректального анастомоза значительно увеличивает послеоперационную летальность. Наиболее эффективным способом профилактики грозных осложнений является формирование превентивной кишечной стомы, однако показания, выбор конкретной методики и сроки удаления стомы четко не определены.
