Despite the obvious advances in the healthcare related to the diagnosis of the disease and its treatment, oncopathology occupies a high position in the structure of the overall morbidity. <italic><bold>Clinical case description:</bold></italic> Patient T., 62 years old, came to an internist with the complaints of constant pulling pains in the lower abdomen, a stool disorder in the form of alternating constipation and diarrhea, presence of blood in the stool over the past 6 months, a weight loss of 8 kg over the past 4 months. She was examined independently two years ago, when the disorders of the gastrointestinal tract appeared for the first time, which she associated with constant stressful situations at work. According to the study results, no significant deviations were found. The patient also underwent 16s-RNA sequencing of the intestinal microbiome, which revealed significant changes in many clusters of bacteria (a decrease in the quantitative composition of Lactobacterium, Bifidobacterium, Akkermansia, Faecalibacterium, which is the main representative of the normal intestinal flora, that may indicate a decrease in the protective barrier function of the colonic mucosa, an increase in Fusobacterium, as well as other representatives of the pathogenic flora). Presently, within the framework of laboratory and instrumental studies, the following diagnosis was established: "Malignant formation of the colon (colorectal cancer). T3M0N1". A repeated study of the intestinal microbiota was carried out, according to the results of which an increase in Fusobacterium up to 7.5% was observed, along with a sharp decrease in the representatives of the normoflora (&lt;1%). <italic><bold>Conclusion:</bold> </italic>Changes in various bacterial clusters of the intestinal microbiota can serve as an early marker for the verification of malignant neoplasms of the large intestine. Determining the amount of Fusobacterium in the intestinal microbiome can be an indicator of the first diagnostic line, which helps to identify the pathological process at the stage of precancer, take timely therapeutic measures, improve the quality of life, and increase its longevity.</p></abstract><trans-abstract xml:lang="ru"><p><italic><bold>Обоснование.</bold></italic> В настоящее время фиксируется рост онкологических заболеваний среди лиц пожилого возраста. Несмотря на явные успехи здравоохранения, связанные с диагностированием болезни и ее лечением, онкопатология занимает высокие позиции в структуре общей заболеваемости. <italic><bold>Описание клинического случая.</bold></italic> К терапевту с жалобами на боли внизу живота, чередующиеся запоры и диарею, присутствие крови в кале на протяжении последних 6 мес, снижение веса на 8 кг за последние 4 мес обратилась 62-летняя пациентка. Жалобы на расстройства желудочно-кишечного тракта появились два года назад, обследовалась самостоятельно, по результатам исследований значительных отклонений не выявлено. В рамках диагностического поиска пациентке было выполнено 16s-РНК секвенирование микробиома кишечника, обнаружившее значительные изменения во многих кластерах бактерий (снижение количественного состава Lactobacterium, Bifidobacterium, Akkermansia, а также Faecalibacterium, являющейся основным представителем нормальной флоры кишечника, что может указывать на снижение защитной барьерной функции слизистой, увеличение Fusobacterium до 1,3%, а также других представителей патогенной флоры). В рамках лабораторно-инструментального обследования в настоящее время был поставлен диагноз: «Злокачественное образование ободочной кишки (колоректальный рак). T3M0N1». Проведено повторное исследование кишечной микробиоты, по результатам которого наблюдалось резкое снижение представителей нормофлоры (&lt;1%) и увеличение Fusobacterium до 7,5%. <italic><bold>Заключение.</bold></italic> Изменение различных кластеров бактерий кишечной микробиоты может служить ранним маркером для верификации злокачественных новообразований толстого кишечника.
