The most formidable and severe complication in morbid obesity patients after gastric bypass is gastrojejunostomy leak.</p> <p><bold>Aim:</bold> to study the possibilities of improving treatment outcomes of obese patients during gastrojejunostomy leak after gastric bypass surgery using new minimally invasive methods.</p> <p><bold>Methods:</bold> This retrospective cohort descriptive study included all patients with gastrojejunostomy leak after gastric bypass treated in the Department of Surgery of the Federal State Budgetary Institution Federal Scientific and Practical Center of the Federal Medical and Biological Agency of Russia in 2019–2022. In total, 445 gastric bypasses were performed during this period, and seven patients (1.6%) developed HEA failure in the postoperative period.</p> <p><bold>Results:</bold> The results of treatment of gastroenteroanastomotic leaks during gastric bypass in patients with morbid obesity are presented. The early diagnosis of gastroenteroanastomosis failure in this category of patients was difficult. All doubts in the diagnosis of anastomotic failure should be completed by revision laparoscopy with intraoperative esophagogastroscopy. Minimally invasive techniques (surgical, endoscopic), drug therapy, and adequate nutritional support are crucial for managing patients with anastomotic leaks.</p> <p><bold>Conclusion:</bold> The use of an endoscopic intraluminal vacuum aspiration system in the treatment of gastroenteroanastomotic insufficiency avoids the spread of inflammation in the abdominal cavity, accelerates the reparative process in the area of insufficiency, and, in most cases, avoids programmed sanitation, significantly improving the treatment results.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Гастрошунтирование по методу Ру является основным видом бариатрической операции. Наиболее грозным и тяжело протекающим осложнением у пациентов с морбидным ожирением после гастрошунтирования остаётся несостоятельность гастроэнтероанастомоза.</p> <p><bold>Цель исследования</bold> — изучить возможности улучшения результатов лечения пациентов с ожирением при возникновении несостоятельности гастроэнтероанастомоза после операции гастрошунтирования с помощью применения новых минимально инвазивных методов.</p> <p><bold>Методы.</bold> Представлено ретроспективное когортное описательное исследование, включившее всех пациентов с картиной несостоятельности гастроэнтероанастомоза после гастрошунтирования, получавших лечение в отделении хирургии ФГБУ ФНКЦ ФМБА России в 2019–2022 годах. Всего за этот период выполнено 445 операций гастрошунтирования, у 7 (1,6%) пациентов в послеоперационном периоде развилась несостоятельность гастроэнтероанастомоза.</p> <p><bold>Результаты.</bold> Представлены результаты лечения несостоятельности гастроэнтероанастомоза при операциях гастрошунтирования у пациентов с морбидным ожирением. Отмечена сложность ранней диагностики несостоятельности гастроэнтероанастомоза у данной категории пациентов. Все сомнения в диагностике несостоятельности анастомоза должны завершаться ревизионной лапароскопией с интраоперационной эзофагогастроскопией. Применение минимально инвазивных методик (хирургических, эндоскопических), лекарственной терапии, адекватной нутритивной поддержки является неотъемлемым принципом современной тактики ведения пациентов с несостоятельностью анастомозов.</p> <p><bold>Заключение.</bold> Использование эндоскопической внутрипросветной вакуумной аспирационной системы в лечении несостоятельности гастроэнтероанастомоза позволяет избежать распространения воспаления в брюшной полости, ускорить репаративный процесс в зоне несостоятельности и в большинстве случаев избежать программируемых санаций, значительно улучшив тем самым результаты лечения.</p></trans-abstract><kwd-group xml:lang="en"><kwd>morbid obesity</kwd><kwd>gastroenteroanastomosis failure</kwd><kwd>gastric bypass surgery</kwd><kwd>endoscopic intra-luminal vacuum aspiration system</kwd><kwd>minimally invasive methods</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>морбидное ожирение</kwd><kwd>несостоятельность гастроэнтероанастомоза</kwd><kwd>гастрошунтирование по Ру</kwd><kwd>эндоскопическая внутрипросветная вакуумная аспирационная система</kwd><kwd>минимально инвазивные методы</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Borraez-Segura B, Orozco-Hernández JP, Anduquia-Garay F, et al.
