Systems such as RAPID (iSchemaView/RapidAI, USA) and Olea Sphere (Olea Medical, France) are widely used in clinical practice; however, a validated domestic system for automated CT perfusion processing is lacking in the Russian Federation. Among similar systems that have a registration certificate in the Russian Federation, Vitrea (Canon Medical Systems, USA, Japan) is available in clinical practice. <bold>AIM:</bold> To validate the Russian system ArchiMed PRO Chronos by comparing it with the Vitrea (Canon Medical Systems, USA, Japan) software for quantitative assessment of ischemic core and critical hypoperfusion area. <bold>METHODS:</bold> A retrospective study included 52 patients with acute ischemic stroke caused by large vessel occlusion and symptom onset within 24 hours. Correlation between systems was assessed for ischemic core volume and critical hypoperfusion area determination. <bold>RESULTS:</bold> Strong correlation was found between systems for ischemic core volume (r=0.95) and critical hypoperfusion area (r=0.85). After excluding studies with inadequate segmentation, correlation improved to r=0.98 and r=0.93, respectively. Adequate segmentation was achieved in 84.6% of cases for both systems. No statistically significant differences between systems were detected (p &gt; 0.05). <bold>CONCLUSION:</bold> The ArchiMed PRO Chronos system demonstrated excellent agreement with Vitrea and can be used for quantitative CT perfusion analysis and patient selection for reperfusion therapy.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Автоматические системы анализа исследований с использованием метода перфузионной компьютерной томографии (КТП) критичны для отбора пациентов на реперфузионную терапию при остром ишемическом инсульте. Зарубежные системы, такие как RAPID (RapidAI, США) и Olea Sphere (Olea Medical S.A., Франция), широко применяются в клинической практике, однако в Российской Федерации отсутствует валидированная отечественная система для автоматизированной обработки исследований методом КТП. Среди аналогичных систем, имеющих регистрационное удостоверение на территории Российской Федерации, в клинической практике доступна Vitrea (Canon Medical Systems, США, Япония). <bold>Цель исследования</bold> — провести валидацию российской системы АрхиМед PRO Chronos путём сравнения с системой Vitrea (Canon Medical Systems, США, Япония) при количественной оценке ишемического ядра и области критической гипоперфузии. <bold>Методы.</bold> В ретроспективное исследование включено 52 пациента с острым ишемическим инсультом, вызванным окклюзией магистральной артерии, и временем начала заболевания до 24 часов. Производилась оценка корреляция между системами при определении объёма ишемического ядра и зоны критической гипоперфузии. <bold>Результаты.</bold> Выявлена сильная корреляция объёма ядра (r=0,95) и зоны критической гипоперфузии (r=0,85) между системами. После исключения исследований с неадекватной сегментацией корреляция улучшилась до r=0,98 и r=0,93 соответственно. Адекватная сегментация достигнута в 84,6% для обеих систем.
